|a The therapeutic role of surfactant in children suffering acute respiratory failure of different aetiology is still unclear. Actually, surfactant dysfunction appears to be an important contributor in paediatric pulmonary diseases causing acute hypoxemic respiratory failure, such as ARDS and bronchiolitis. Thus, surfactant administration might be a valid therapeutic option in these critically ill patients. Despite several studies have shown positive effects of exogenous surfactant, improving important outcomes such as oxygenation, duration of mechanical ventilation, length of intensive care unit stay, as well as mortality, some other trials have not confirmed such beneficial effects.
